Qualifications
- Board certified and hold a valid state registration or license.
- 3 years of clinical experience in primary care or a related specialty
- Strong understanding of value-based care principles, including risk stratification, care coordination and population health management.
- Current and unrestricted DEA certificate
- Must maintain certification of BLS and have current course credentials.
- Is proficient in health information technology and electronic health records.
Ideal Candidate
- Has excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate effectively with interdisciplinary teams.
- Maintains commitment to continuous learning, quality improvement and patient-centered care.
- Has experience with value-based care contracts, payer relationships and quality reporting preferred.
- Can work without direct supervision and practice autonomously
